Samuel Jakubek of Duluth, Minnesota was inducted into the Army on April 13, 1942 at Fort Snelling, Minn.

He was a Technician 5th Grade and served as a truck driver in Papua, New Guinea and the Southern Philippines.

Mr. Jakubek received the Good Conduct Medal, Asiatic-Pacific Campaign Medal with three bronze battle stars, Philippine Liberation Medal with one bronze star and six overseas service bars.

He was honorably discharged on November 1, 1945 at Camp McCoy, Wis.

"Cpl. Samuel Jakubek, Army medical corps, whose wife lives at 428 North Seventh Avenue East is in New Guinea.

"Pvt. Benjamin, field artillery, is at Fort Sill, Oklahoma. His wife resides at 338 South First Avenue East.

"Pvt. Chester, Army Infantry, is at Camp Fannin, Texas.

"Pvt. Frank, whose wife lives at 274 South First Avenue East is at Fort Benning, Georgia, with the field artillery.

 "Pvt William, whose wife lives at 228 Eighth Avenue East. is at Fort Leonard Wood, Missouri "
